Luthier’s Receipt for Carbonless Copying:

These are half-sheet forms that I designed and have been using to take notes while the guitar is
on the bench. At the end of the job, I calculate the total cost for the job and tear off the yellow carbon
copy for the customer.

This Luthier’s Receipt Template was created to save time writing receipts for customers. I used
to take notes on one sheet that I’d keep in my customer files, then I’d write what I did (again) on a
generic carbonless form. I eventually discovered Carbonless.com and learned that I could have my own
design of carbonless forms custom printed for a lower cost per receipt. I’m loving them and wanted to
share them with the community.

I do a lot of setups for extended range electric guitars, hence the need for 10 strings on my
action chart. I do have a few customers who are extremely meticulous about their setups, so I started
logging “before” and “after” measurements – that way, if they didn’t like something about the way their
guitar played after they pick it up, I could either show them that the guitar had settled in and needed
readjusting or tweak the setup to their liking. The latter almost never happens, but I think it’s cool to
have the data available. Readjusting does need to happen from time to time, and I find myself referring
to my notes.

If you like these forms, the free template is available for download at my website
www.TheGuitarGentleman.com You can have them printed on carbonless forms like I do, or just print
and use them as is.
